Please visit your community’s website and either download the ACC application form or submit your modification request online. If you have any questions please give our office a call or email us at manager@havencm.com.

Please note that it can take up to 30 business days for application decisions to be made.

If submitting a paper application, please be sure it is signed by the homeowner.

The form is filled out in its entirety. Incomplete forms will result in automatic denial and need to be resubmitted.

Please be thorough in the details of your request. If applicable include:

Plans, plats/surveys, and diagrams

Color swatches and material types

Plant type and gallon size

City permits
